You will build relationships with service users, families and staff to contribute to the positive well being of those in your care. You will support people to be independent, feel valued and support them to be a part of their communities. You will ensure that people are supported in daily tasks in a respectful way and aim to meet their individual needs. You will ensure all plans and records relating to people are accurate and up to date. You will key work with individuals to develop their support plan through the referral, assessment and review stages through person centred planning.
What youll need to succeed
You will need to be computer literate. This job is 37.5 hours per week, and is a 24/7 service, so you will need to be able to work flexibly across the week. You will have experience in working with individuals learning disabilities and building and maintaining relationships with them.
